Case Management Coordinator

This role is work at home following up to 5 days of office based training in Tampa, FL
50-75% of travel is required around Highlands County.
Utilizes critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources.
Fundamental components include but are not limited to:
- Conducts comprehensive evaluation of Members using care management tools and information/data review
- Coordinates and implements assigned care plan activities and monitors care plan progress
- Conducts multidisciplinary review in order to achieve optimal outcomes
- Identifies and escalates quality of care issues through established channels
- Utilizes negotiation skills to secure appropriate options and services necessary to meet the members benefits and/or healthcare needs
- Utilizes influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health
- Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices
- Helps member actively and knowledgably participate with their provider in healthcare decision-making
Monitoring, Evaluation and Documentation of Care: Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.
Qualification Requirements & Preferred Skills:
- A Bachelor's Degree in social work or related field is required.
- A minimum of 2 years experience in behavioral health or social services required.
- Managed Care experience preferred.
- Long Term Care experience preferred.
- Case management and discharge planning experience preferred.
- Bi-lingual preferred (Spanish/ English).
- Candidate must have computer data entry knowledge.
- A valid driver's license is required.
The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
Telework Specifications:
Full-Time Telework (WAH) with office support location in Tampa, FL (3611 Queen Palm Drive).
Required in-office training before transitioning to WAH.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
